1. Scope of Services
White Oak Dental provides general and specialty dental care services, including but not limited to routine check-ups, cleanings, fillings, root canals, extractions, crowns, bridges, implants, dentures, teeth whitening, and emergency dental care. All services are rendered by licensed dental professionals in accordance with applicable laws and regulations.
2. Appointment & Cancellation
Appointments are scheduled on a first-come, first-served basis. We request that you provide at least 24 hours' notice for cancellations or rescheduling. Failure to do so may result in a cancellation fee. White Oak Dental reserves the right to reschedule or cancel appointments due to unforeseen circumstances, with timely notice to the patient.
3. Payment & Insurance
Payment is due at the time of service unless prior arrangements have been made. We accept cash, credit/debit cards, and digital payments. For CGHS beneficiaries, treatment is provided at CGHS rates upon presentation of a valid CGHS card and referral letter. It is your responsibility to verify your insurance or empanelement coverage before treatment.
4. Patient Responsibilities
Patients are expected to provide accurate and complete medical and dental history information. White Oak Dental is not liable for complications arising from undisclosed pre-existing conditions, failure to follow post-treatment instructions, or missed appointments. Patients must inform the clinic of any changes to their medical history or medications prior to each visit.
5. Treatment Consent
By signing the consent form, you acknowledge that you have been informed about the proposed treatment, its risks, benefits, alternatives, and associated costs. You have the right to ask questions and withdraw consent at any time. No invasive procedure will be performed without your explicit written consent.
6. Limitation of Liability
White Oak Dental shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services. Our total liability for any claim shall not exceed the amount paid by you for the specific service giving rise to the claim.
7. Changes to Terms
White Oak Dental reserves the right to update or modify these Terms & Conditions at any time without prior notice. Changes will be effective immediately upon posting to this page. We encourage you to review this page periodically.
